Patients in Carlton County are affected by disparities that create unique barriers to health. Over 30% of the population suffers from hypertension and patients often require multiple medications. Medication Therapy Management (MTM) services were instituted within the pharmacy to improve patient referral from the clinic, improve communications between the pharmacist and prescribers, and ultimately improve patient outcomes.
Ryan Harden, MD, MS, board certified in Family Medicine, practices at Gateway Clinic in Sandstone, Moose Lake and Hinckley, MN, and Essentia Health-Sandstone. He serves as Medical Director at Gateway Clinic and Essentia Health-Sandstone. Dr. Harden is a faculty member at UMD Medical School and serves as course director for Community Clinical Medicine. He also teaches Biochemistry, Molecular Biology, Medical Genetics and Pediatrics with Kaplan Medical and precepts at Duluth Family Medicine Clinic.
Kendra Metz, PharmD, graduated with honors from the University of Minnesota College of Pharmacy and is the pharmacist-in-charge at Thrifty White at Gateway Clinic in Moose Lake, MN. Kendra values patient education and prevention and provides MTM as well as other services. She has developed good working relationships with the prescribers and staff at Gateway Clinic, implementing collaborative practice agreements. She has the support of Thrifty White leadership in her innovative clinical endeavors.
Sarah Nelson, MD, is a clinical consultant with Healthy Northland. She has worked with primary care clinics to improve quality for chronic disease prevention through several public health grants. She has experience in primary care practice in rural Minnesota and in working with healthcare, public health and community organizations to increase clinic - community linkages for better patient care.
