The Renville County Hospital and Clinics in Olivia are taking a multi-sector approach to advance health equity and address health-related social needs through community collaborations that address pain, healing, nutrition, movement, and social connectedness. By approaching wellness from a broad perspective, our community seeks to integrate cultural healing and health care to better achieve whole-person and community well-being. This breakout will highlight: strategies to transform data use by pairing comprehensive data across clinic, public health and community wellness reports; facilitation methods used to enlighten collaboration as an equitable process; vertical and horizontal community collaboration; and initial outcomes.
Samantha Nelson, MEd
Samantha's career began in public education transitioning through her passion for public health and health equity health care reform in Southwest Minnesota. She worked across the region with local champions addressing population health needs. Currently, Samantha leads community wellness and outreach work at Renville County Hospital and Clinics with health equity driving collaboration and a vision for sustainable personal and community well-being. All people have the right to experience personal wellness and culturally appropriate healing modalities.
