French River 1/2 Room

HealthFinders Collaborative (HFC) began as a free clinic in a church basement. Developed outside of traditional health systems, HFC was able to create innovative services for some of Minnesota’s most diverse populations. HFC embarked on an innovative multi-year care coordination initiative that successfully built clinical and community partnerships. They have become a thriving health center with initiatives that span the continuum of care. Medical, dental, wellness, and community engagement programs make up HCF’s unique care coordination programs. In this breakout, HFC will present their program evaluation studies, clinic outcomes, and how they plan to strengthen their position and partnerships in the transition to value-based care.

Since 2010, Charlie has been the Executive Director of HealthFinders Collaborative, leading the organization through a critical stage of growth. He is passionate about engaging communities in their own health and rethinking primary care to be more responsive to local communities. He has studied public health systems around the world, received his Master's of Public Health from the University of Minnesota, and currently serves as chair of the Northfield Hospital and Clinics Board.

Colin Cureton

Colin Cureton, MS, MPP

Cor Findings, LLC

As Principal of Cor Findings, LLC, Colin provides research and evaluation expertise at the intersection of health, economics, and food systems. In these and previous roles, he has worked with many health systems and entities to advance community-based health solutions. He holds a Masters of Science in Applied Economics and a Masters of Public Policy in Advanced Policy Analysis from the University of Minnesota. He and his family live in Saint Paul, Minnesota.
